Citizens Choice Healthplan HMO has contracted with many hospitals throughout Los Angeles, Northern Orange County and Inland Empire to provide its members with access to quality care and service throughout their community.

You must use plan providers except in emergent care situations. If you obtain routine care from out-of-network providers neither Medicare nor Citizens Choice Healthplan will be responsible for costs.
