Citizens Choice Healthplan HMO has contracted with many hospitals throughout Los Angeles and Northern Orange County to provide its members with access to quality care and service throughout their community. As a member of Citizens Choice Healthplan your assigned primary care physician will let you know their hospital affiliations.

You must use plan providers except in emergent or urgent care situations. If you obtain routine care from out-of-network providers neither Medicare nor Citizens Choice Healthplan will be responsible for costs.
